Most ovarian cysts are harmless fluid-filled sacs that disappear on their own. Surgery is needed only when a cyst is large, persistent, causing symptoms, or shows features that need closer assessment. When removal is required, it is usually done by keyhole surgery, often preserving the ovary.
What is an ovarian cyst?
An ovarian cyst is a fluid-filled sac on or in an ovary. Many form as part of the normal menstrual cycle and resolve within a few months. Others — such as endometriomas or dermoid cysts — may persist and need treatment.
When is surgery needed?
Your physician may recommend cyst removal if a cyst is large, growing, causing pain, twisting the ovary, or has features on ultrasound that warrant removal. Many smaller, simple cysts are simply monitored with repeat scans.
How the operation is done
- Cystectomy — the cyst is removed while the healthy ovary is preserved (the usual goal, especially for younger women).
- Oophorectomy — removal of the ovary, considered only when necessary.
Both are most often performed laparoscopically (keyhole), through a few small incisions with a camera.
Recovery
After keyhole surgery, many women go home the same day or after one night and return to light activity within one to two weeks. You will have written aftercare instructions and a follow-up appointment.
Will it affect my fertility?
Removing a cyst while preserving the ovary is designed to protect fertility. Your surgeon discusses the specific impact on your ovarian reserve before the operation.
